application forms for a family break in 2008 are
no longer available for
download.
Please note that applications are only accepted from
welfare agents*
and not
directly from families.
*Welfare agent is the term we use to describe
social workers, health visitors, schools, GPs,
other charities who can receive monies on behalf
of the family, support them in preparation of the
holiday and provide feedback following the break.

We consider such applications on a ‘first come, first served basis’
and anticipate that welfare agents
will begin to be notified between
January and April 2008 - refer to the Frequently Asked
Question sheet below. Grants will only be
allocated if our
criteria are met and if funds are available.
All applicants need to meet the following
criteria

The
family is on a low income
-
The
family has not been on holiday for the
past four years
-
The
family has at least one child of three
years of age or over
-
The
family has been referred by a welfare
agent
